The clinical partner explains who provides care, what the program includes, available support, expected costs, how information is handled and the applicable consent process. A public website inquiry is not clinical enrollment.
Participation and permission
Ask how to pause or stop routine calls, change contact or language preferences, and authorize or remove family involvement. The team explains the process and any effect on the care plan.
Clinical records and privacy notices
The responsible clinical partner provides the applicable Notice of Privacy Practices and processes requests relating to clinical records. KindlyMoments’ website privacy notice does not replace that document.
